ABSTRACT:
Background:Frozen shoulder was explained first as “periarthritscapsulohumeral” in 1834(1, 2), in 1934 frozen shoulder was described as a condition in which pain is felt near the deltoid insertion along with limitation of moment and unable to sleep on affected side but radiological findings are normal(3).Frozen shoulder has four stages i.e. stage 1(very painful), stage 2(freezing stage), stage 3(frozen stage), stage 4 (thawing stage)(4).
METHODOLOGY: A survey study in which non-probability sampling technique were used. 112 physiotherapist were given questionnaire and data was collected. The study duration was 6 months.
RESULTS: Total 112 Physiotherapists (58% male and 42% female) were included. For diagnosis 84% were looking for limitation of movement, 77% for capsular pattern, and 65% for shoulder pain. X-ray was used forincluding or excluding bony abnormality (57%), osteoarthritis (43%), fractures (36%) and calcific tendinitis (32%). Freezing stage treatment included superficial heat and cold therapy (69%), gentle active exercises (62%), electro therapy (59%), advice and education (56%), hands on soft tissue technique (52%), and mobilization (49%). Stage2 and 3 treatment was mobilization (78%), function based exercise (57%), hands on soft tissue technique (52%), electro therapy (47%), and sustain stretching exercise (46%). Majority used medicines.Ultrasound (72%), TENS, short wave diathermy and combination therapy were 54%, 51%, and 20% respectively.
CONCLUSION: The study shows that diagnosis protocol is followed by Peshawar physiotherapist but radiographic data showed be asked to exclude red flags. However freezing stage treatment is different from international protocol, while stage 2 and 3 treatment is similar to international protocol. Further study is needed in to find out number of participants who follow international protocol to its full potential and how do they cope with patient’s depression during all sessions
