Background: Menstruation is a natural monthly cycle in every female which is the shedding of endometrium lining. During menstruation many hormonal changes occurs in female body which might affect their other system.

Objective: To determine the effect of menstrual cycle on cardiorespiratory fitness and psychological effect in females attending fitness center.

Methodology: Comparative cross-sectional study was conducted on menstruating females with sample size of 264 which were divided in two groups in which 132 were active females who were attending gym regularly and 132 sedentary females.  Harvard step test was applied on them to measure their cardiorespiratory fitness. Two scales were used BORG scale to measure their exertion after exercise and perceived Stress scale to check their stress level. The data was analyzed and presented in form of mean, standard deviation, frequency, percentage, test of normality (Shapiro-wilk) and Mann-Whitney test by using SPSS version 27.

Results: In the study the Harvard step test shows that the menstrual cycle has no such effect on cardiorespiratory in 132 active group (Mean rank=83.21) as compared to 132 sedentary group (Mean rank=187.79). By using BORG scale of perceived exertion, it was shown that the exertion level in active group were less (Mean rank=77.44) as compared to sedentary group (Mean rank=187.56). In sedentary group the Perceived stress scale (PSS) was greater, and their stress level was high (Mean rank=143.87) as compared to active group (Mean rank=121.14) in which stress level was low.

Conclusion: It was concluded that there is significant difference between two groups active and sedentary group. The menstrual cycle has no such effect on cardiorespiratory fitness in females attending fitness center. There is moderate level of stress in active group but as compared to sedentary level the stress level is low.
